As part of your care, we’ll schedule you for an Annual Health Assessment and Screening to check in on your overall well-being. This visit helps us identify and address any health concerns early, before they become more serious. Our approach is proactive, personalized, and designed to keep you feeling your best for the long term.

Your Transition of Care Coordinator
Lauren partners closely with patients, their families (as defined by the patient), and staff across all departments to ensure smooth transitions of care. She coordinates services during important moments such as ER discharges, hospital discharges, palliative care, hospice, and home health. Her role is to help create a seamless experience so patients feel supported and connected every step of the way.

Leticia is dedicated to helping patients confidently navigate the healthcare system. She supports patients and families by coordinating care, clearly explaining treatment options, and assisting with insurance or billing questions. With compassion and attention to detail, Leticia ensures that each patient’s needs and preferences are respected. Acting as a trusted bridge between patients, families, and healthcare providers, she works to make the care experience smoother, clearer, and more supportive.
