At Citrus Valley Medical, we’ve built a dedicated care team focused on supporting your overall health and well-being. Our Care Coordinators work closely with your provider to guide, assist, and advocate for you every step of the way, helping you stay healthy, informed, and confident in your care.
Through our Coordinated Care Clinic, we identify and address potential health concerns early, often before they become serious. This coordinated approach helps reduce unnecessary emergency room visits and hospital admissions, allowing you to remain safely and comfortably at home whenever possible.

We’re behind the Walgreens on Main and Ontario Ave. Classes typically last 60 minutes.

Your Transition of Care Coordinator
Lauren partners closely with patients, their families (as defined by the patient), and staff across all departments to ensure smooth transitions of care. She coordinates services during important moments such as ER discharges, hospital discharges, palliative care, hospice, and home health. Her role is to help create a seamless experience so patients feel supported and connected every step of the way.
